• Referral slip. This patient referral form is provided for the use and convenience of physicians and their office personnel only. The slip contains all the necessary information to refer a patient for bone density testing at the OOC.

• CURRENT CLINICAL RESEARCH STUDIES

Dear Colleague:

Our clinical research study has actively evaluated new treatments and treatment regimens for osteoporosis for the past 12 years. We are currently enrolling subjects in several clinical research studies. The evaluation of subjects for study entry, including bone density testing, routine laboratory studies and study medications are provided at no cost to the participants. Being a part of one of our studies does not interfere with the relationship between the research subjects and his or her primary care provider. We work diligently to communicate regularly and effectively with the subject's physician, and we forward the results of study laboratory procedures to the health care provider at regular intervals.
